How do I become a new patient with Wellness By Design?

Wellness By Design operates on a Membership based model of care designed to maximize patient outcomes and minimize the hassles and inconveniences rampant in fee for service medicine. There are two options for becoming a patient here:

  1. If you feel that your symptoms are mainly related to low hormones, please Click Here
  2. If your symptoms are more far-reaching such as severe fatigue or other general health issues, then we invite you to attend the next seminar to hear the full spectrum of illnesses that Dr. Connie can help with.
Can Dr. Carver be my primary care provider?

Dr. Carver is an Integrative Medicine specialist and does not maintain hospital privileges or operate as a PCP. We recommend that you maintain your relationship with a PCP who accepts your insurance, provides same-day service, and can be on-board with your specialized care here at Wellness By Design.

Do you accept insurance?

As an Integrative Medicine Practice we do not participate with any insurance programs, nor are we listed on any insurer’s preferred provider list. Integrative Medicine is still considered “alternative” medicine and does not fall under the restrictive practices that insurance companies dictate. We are considered “out of network” so look at your policy under ‘out of network’ provisions to see what they may cover. Most likely you have a separate deductible, and the insurance company may pay a percentage of the portion that is actually covered. We are happy to provide you with a form containing all of the diagnostic codes and receipts for procedures your insurance carrier may reimburse for. There is a $25 charge for each form prepared.

WBD accepts all forms of Medical Spending Accounts.

What about Medicare?

Many Lab tests can be billed to Medicare so long as there is a valid diagnosis that requires the test. Patients must sign an agreement not to bill Medicare for services received at Wellness By Design, since we have “opted out” of Medicare.

Do you know of any doctors close to my home who are similar to your practice?

Dr. Casebolt Carver belongs to several professional organizations that promote Integrative or Functional Medicine. Most of these maintain a search tab for like-minded doctors. These organizations include functionalmedicine.orgACAM.orgICIM.org and oxygenhealingtherapies.com.
